The Sawai Man Singh (SMS) hospital Jaipur India doctors conducted the first skin transplant and gave new life to a 14-year-old boy, who had suffered 40% burn injuries due to electrocution.

Fourteen-year-old Vishnu became the first recipient of cadaver skin transplant. For the past three months, he has been undergoing treatment in SMS hospital. But, the defining moment in his life came when doctors decided to give him a skin transplant, the first-of-its-kind in the hospital.

Cadaver Liver transplant preparation in progress at SMS. ILBS head meets the medical minister Dr. Girish, Chief at Institute of Liver and Bleary science met the medical minister at his residence along with a team of specialty doctors. Mr. Rathore, Medical minister has given his directions to start a team of four specialist doctors, i.e. two from ILBS team and two from SMS under the head of SMS medical college and to start the work of liver transplant as soon as possible.

According to MOU, ILBS team would do the first 25 transplants and then later SMS doctors would take up the responsibility. He has asked to start the facility of both cadaver liver and live liver transplant.

In Special team – Dr Ajay Sharma, Dr Randhir Singh from SMS and DR Girish and one other specialist from ILBS team would be under the special team force. The medical minister took necessary information about the process of liver transplant and cost from Dr Girish and gave orders to the dean of SMS College to buy the needed equipment.

We have heard of Kidney and Gall Bladder stones but this is one of the first cases in India of stones in Brain. A women aged 20 years of Jhunjunu has been diagnosed with Albright's hereditary osteodystrophy.

It happens in 3 out of ten lakh people. The women’s epilepsy episodes have been fully controlled through medication. According to DR U.S Jain of SMS Medical College, the rare disease thesis written by DR RS Jain of neurology department at SMS has been published in the British medical journal under the name Brain Stones.

According to Dr. RS Jain the women from Jhunjunu was suffering from epilepsy since last 10 years along with problems of stressed nerves of hands and feet and extreme pain. After diagnosis lack of calcium, and phosphate and Para thyroid in high numbers came forward which are main reasons for brain stones. She also had glaucoma eye surgery in the age of 18 years. After the treatment, she has recovered.

Eye Cornea Endothelial transplant Surgery

Successful Eye Cornea Endothelial transplant Surgery of two patients at SMS Hospital Jaipur

The doctors of eye department at SMS hospital have successfully operated the cornea of two patients and completed endothelial transplant. This is also known as Descemet membrane keratoplasty. 

The operation lasted for 2 hours and the patient can now see properly. According to Dr. Dharamveer Singh, the innermost layer of cornea, which is like a cell membrane, is taken out and transplanted. He further adds that this is one of the most delicate of the surgeries performed until now. This operation was done with 2 to 3 mm hole. Dr Shashi and other doctors supported this operation.

Post care location for liver transplant

Post care location for liver transplant in SMS Jaipur finalized

Work is in progress for Liver transplant process at SMS Jaipur. The first stepping-stone has been placed to make the process of liver transplant easier. The SMS medical college and Hospital authorities have marked the location for Post care unit for liver transplant. The budget is also finalized and approved. The work for liver transplant post care unit build would start in a few days.

Three units of post care would be built near the gastro operation theatre of Dhanvantri department. The work will be completed in three months. With the onset of post care unit the process of liver transplant will commence at SMS hospital. Earlier the liver transplant process was finalized at SMS, but the ILBS team from Delhi rejected it in absence of post care unit.

Will not take risks in Liver Transplant-

According to Dr. U.S. Agarwal, Principal SMS Medical College, The post care unit for liver transplant is very necessary at SMS. The location has been marked and the work shall start shortly. We do not want to take any risks in Liver transplant so all the necessary requirements will be met first.

Decisions taken at SMS meet-

At the meeting on Wednesday regarding organ donation the authorities decided that doctors would work extra hours regarding organ donation process. All senior, junior doctors and staff would provide extra time for the same. The NGO’s and staff should be involved more in counseling when the case of brain dead arises. The meeting was conducted under the guidance of Organ donation head, Dr. Vijay Tomar.

The radiologists of city are suffering from diseases in lieu of treatment of patients. No hospital seems to have record of staff that is sick due to radiation. Many are opting out of this career due to less salary and no safety protocols. Many radiologists have acquired diseases due to harmful effects of radiation. There have been many cases in which even their younger generation seemed to be suffering from the radiation effects.

Despite all this, no safety equipment have been arranged for the radiologists. Now no one seems interested in this job and especially the upcoming generation. 

According to Mumbai’s nuclear energy, board a radiologist can test up to 20 to 25 cases per day. However, because of free checkups there is work pressure of about 80 to 100 x rays in a day. According to rules, a worker cannot be kept for more than 40 hours in a week in radiation zone.

As per records of SMS hospital, 50% radiation workers out of 600 are suffering from baldness, cancer, leukemia, irrtatibility, skin problems in legs, abortion, eye, and birth defects. 

According to Dr. Anand Verma, former head of radio diagnosis department of SMS medical college, the radiation workers are working in the face of death. Institutes should research and find the causes and act on curbing of diseases.

Separate ICU for Liver transplant in SMS

One person would save five others in future.

Separate ICU would be made for Liver transplant at SMS Jaipur-

Rajasthan is moving forward in the positive direction of organ transplant. After successful Cadaver kidney and other organ transplants, the next step is in Liver transplants. SMS hospital will have a new ICU for the same. After the instructions of Medical Minister Mr. Rajendra Rathore, the work has been commenced on college level. The ICU is expected to be ready in forthcoming months. Patients with Liver transplants would be admitted to this ICU. As Cadaver, liver transplant requires special after care of patients thus this decision was taken for future.

Also after many years, the Cadaver kidney transplant is also on the path of progress. Eight cadaver kidney transplants have been performed in the state. In addition, the state government has announced to make an organ transplant institute at SMS hospital Jaipur.

The growth and awareness of organ donation-

Earlier even eye donators were very few in Rajasthan. Then people got more and more aware and now this number has increased ten folds. Similar was the case with body donation, but now each year in SMS alone there have been 10 to 15 donation cases. Similar is the scenario of liver and kidney donators. A private hospital and SMS have already started work on the same.

Organs of brain dead remain in working condition-

According to Dr. U.S Agarwal, Prof SMS Medical College, Brain dead is a state when the brain stops functioning. These patients remain on ventilator and it is not possible to save them. However, their organs like kidneys, liver, eyes, pancreas, and heart keep functioning. As per experts, these organs can be used as lifesavers to give new life to injured and sick patients.

Treatment cost decreased-

According to Dr. Ajay Sharma, Gastro Surgeon, SMS, Earlier the patients had to go to other states to get cadaver transplant surgery that costs about 25 to 30 lakhs. Also extra cost for living/food etc of family members is additional. If the same surgery is performed in one’s own state then the cost is decreased by 25 to 30%.After the success of cadaver kidney transplants, SMS will perform liver and heart transplants.

Counseling is necessary-


It takes quite the effort to explain the process and benefits of saving other lives to family members of brain dead patients but the situation is expected to become better. Dr Vinay Tomar, Head of urology, SMS have been appointed as transplant head by the authorities.

SMS OPD over crowded with patients

Doctors unequipped to handle increasing number of patients.

Waiting time increases two – folds for patients.

Doctors at SMS hospital are unable to handle the growing number of patients every day. On and average one doctor has around 150 patients lined up daily. This is also decreasing per patient time with the doctor.



The outdoor facility has seen large growth in the number of patients in last 5 years. However, the doctor to patient ratio is not in equilibrium. According to statistics five years back where there were around 60 patients per doctor, now the number has increased to 155 per doctor. There are 16 clinical departments at the hospital with an average of four doctors in respective OPD’s. Doctor patient time has also decreased in last 5 years. Where earlier, it was approximately 5 minutes per patient and now it has decreased to a minute per patient. 

More than 200 patients per doctor in medicine, skin, and cardiology departments-

According to Dr. Ajit Singh, Additional medical superintendent SMS, “Around 65 doctors are present in one department in OPD. On an average of 10,000 patients, around 625 are present in each department. Although in some departments like cardiology, Medicine, and skin this number increases to more than 1000.This, directly impacts doctor patient time and in such departments, the average number of patients visiting one doctor increases to 200.

According to Dr. U.S Agarwal Lecturer SMS Medical College and Skin Specialist, “The increasing number of patients has put pressure on doctors but still we are maintaining our qualitative standards. Dr Shashi Mohan Sharma, Additional Lecturer SMS Medical College comments, “Many times the number of patients in OPD reaches 200.”

Around 62 children have been benefiting from treatment at State’s largest child hospital JK Loan’s rare disease clinic and all children are on path of recovery now after 5 months. According to ICMR there are more than seven crore cases of more than 7000 rare diseases in India.

In Rajasthan only the number of cases amount to more than 25 lakhs. Any disease which occurs in one out of 2000 people comes under rare diseases according to the European 
council.


Rajasthan is at the top-

Due to less information, knowledge and how of such rare diseases even doctors are sometimes unable to detect them and normal treatments continue which leads to deterioration of health of patient and then the treatment or cure becomes impossible. To eliminate this problem Rajasthan government took a decision to open this rare disease clinic in April. According to Dr. Ashok Gupta of J.K Loan Hospital “The first rare disease clinic to be launched in Rajasthan is a big step in the field of medical science. Around 62 cases have been diagnosed and treated. The main benefit is that the doctors now have the knowledge of such rare diseases which was impossible to find before.

Information about Two Cases – Saved from rare disease

Case 1. An infant child, daughter of Kailash from Barmer, was nearly unconscious and was not responding properly since 3 days. She had problems in both her hands and legs within two days of birth and had stopped drinking milk. Tests like CBC, Serum Calcium and ammonia, renal function, septic screen and MRI etc were performed on the infant and it was deterred that she had metabolism problem. She was treated for same and is healthy now.

Case 2. A child of 11 months had vomiting and diarrhea and within 8 hours the respiratory system started deteriorating. The child was admitted for around 12 days in JK Loan hospital and several tests like ACOT, Cholesterol, Serum Lactate, ammonia etc were conducted and was inferred that the child was suffering from Galactosemia. The new treatment for the same was started and the child is healthy now.

I recently had the opportunity to work in the Indian Government Hospital setting as a transplant surgery adviser to the Government of Rajasthan and it reminded me of the Veteran's Administration Hospitals in the U.S. Any big bureaucracy anywhere in the world is bound to have inefficiencies and strange cultural quirks. When trying to move forward with a complex endeavor such as transplant, there will be resistance and frustrations involved. Here are 10 observations from my experience, most of which now seem amusing but when they were happening in real time, they often made for head-exploding moments. Please note that these comments are not confined to India (I adore this country and respect it in so many ways), they are just examples of goofy bureaucracies worldwide.

1. The longer I keep you waiting, the more important I am.
This is a common power-wielding tactic throughout humanity, but in the strangely plastic time-space warp that exists in India, significant delays and inefficiencies can result. My time is important. Our time is important. Please don't disrespect us to calm your own insecurities.

2. Let me show you how powerful I am by wasting your time.
This is actually a corollary to #1, but deserves its own entry. I was in a meeting where the bureaucrat paused in the middle to dictate a letter to his assistant about what we had just been discussing. Okay, it only took 10 minutes, but c'mon... really?
3. Yes, yes of course (absolutely not).
I was warned about this one before I came to India. Everyone nodding in agreement out of politeness and fear of saying no. Sometimes it takes a while to figure out people's real intentions and much valuable time can be wasted. Please just tell me if you can't or do not want to do it; I will find someone else who is willing to help me.

4. There is no central purchasing department; please navigate the labyrinth yourself.
If you want the government hospital to purchase equipment or supplies on your behalf (that is, if you are lucky enough that they are willing to spend money on you), then you have to do almost everything yourself. Write down each item. Detail the specifications for each. Identify all potential suppliers so they can competitively bid.
You must spend your valuable time doing this instead of saving the lives of your patients... Aargh!
5. I am simply interested in looking good for a promotion or not doing anything controversial before retiring.
Sad but true, most of us will try to position ourselves for advancement or protect ourselves from change when it comes to our jobs. My issue with this is that so many people try to do this without doing any actual work. Those trying to look good will rely on the hard work of others (usually their subordinates) and those trying to stay safe will simply not lift a finger.

6. The 10:00 a.m.-3:00 p.m. shift is charity work -- those patients be damned. Cash flow starts with the private clinic from 3-11 p.m.
Okay, this one is a little harsh and I know that Government doctors in India are not lazy. They are incredibly intelligent (appointment to a Government Hospital is entirely merit based, so they are the cream of the crop) but their pay is very little compared to the private sector. Out of necessity (we all gotta eat!), many government doctors conduct private clinics in their off hours to provide the bulk of their income. Unfortunately, this situation sometimes instills an attitude of less attention toward the government hospital patients because the doctor is not necessarily available 24/7 for emergency consultation.

7. Accountability and transparency are foreign concepts to be resisted.

The lack of accountability and transparency in all aspects of Indian society needs to change if India really wants to be regarded as a serious world stage player. Particularly for a field like transplantation, every active transplant center needs to be answerable to their performance and outcomes relative to everyone else. Only in this way will superior and suboptimal practices be unveiled so that real and steady progress can be achieved. Transparency is also paramount to gain the trust of the public and nontransplant medical professionals.
8. Cherish inertia and shun innovation.
The thought of change can be uncomfortable. But in a country like India where one's very survival depends on a constant state of adaptation to change, I find it curious that maintaining the status quo is as valued as is everywhere else in the world. Sure, people work hard to get to where they are professionally and change can be threatening, but moving forward and trying something new are critical for realizing innovation. And everyone knows that India's great promise as a future world leader depends on her people's innovation and creativity.
9. What is quality?
Quality assurance and outcomes-based assessments are relatively new concepts even in Western medicine, but India cannot afford to ignore these trends. After all, the practice of medicine should really at its heart be all about the patient's well being. Things should not happen just because they can. Attention to quality at every level--from whether a patient lives or dies to whether the family is satisfied with the cleanliness of their loved one's hospital room -- is so important in gaining international respect and attracting business. Oh yeah, it's also best for the patient!
10. Don't challenge my VIP-ness.
I'm still trying to understand the prevalent "VIP culture" here in India. Exaggerated feelings of self importance exist everywhere in this world, but it seems to me that there are more VIP wanna-be's among the Indians (and NRIs) I know compared to my non-Indian friends. As a result, doctors, hospital administrators and politicians are all more prone to having their feelings hurt if they feel slighted or threatened. This is a minefield that deserves respect and attention, but can make the chess game of advancing an agenda so much more complicated.

Finally After 30 years, SMS toppers get gold medals

They were all toppers in their subjects when they graduated from the Sawai Man Singh (SMS) medical college in 1984-85 and in the following years. But their gold medals remained elusive. Thirty years later, on Thursday, these toppers, some of whose children are now studying in the medical college, finally got the chance to bask in their moment of glory after they were given the medal.

"What an amazing feeling it generates when you are awarded with gold medals for academics in the same college where your children are studying," Dr Vinita Patni, a city-based gynecologist said.

एसएमएस में नहीं हो सका लिवर प्रत्यारोपण


सवाई मानसिंह अस्पताल में एक बार फिर लिवर प्रत्यारोपण नहीं किया जा सका। लिवर प्रत्यारोपण के अंतरराष्ट्रीय विशेषज्ञ डॉ.क्रिस्टोफर बैरी की मौजूदगी के बावजूद कैडेवर दानदाता की ओर से दान किया गया लिवर मंगलवार को एसएमएस अस्पताल से ग्रीन कॉरिडोर बनाकर दिल्ली के अपोलो अस्पताल भेज दिया गया। अपोलो के डॉक्टरों की टीम के निर्देशन में इस पूरी कार्यवाही को अंजाम दिया गया। 

बताया जा रहा है कि दानदाता का लिवर सही तरीके से काम नहीं कर पा रहा था, इसलिए यहां प्रत्यारोपण करने की जोखिम नहीं उठाई गई। इसके अलावा प्रत्यारोपण की अनुमति के लिए गठित डॉक्टरों ने भी एसएमएस अस्पताल में इसे अति जोखिम वाला मानते हुए अनुमति नहीं दी।

First cadaver organ transplant at SMS hosipital

GOOD NEWS: एसएमएस में सफल हुआ पहला किडनी केडेवर ट्रांसप्लांट 


जयपुर प्रदेश में किडनी की बीमारी से जूझ रहे मरीजों को अब बड़ी राहत मिलने की उम्मीद है। सवाई मानसिंह अस्पताल में बीती देर रात पहला कैडेबर किडनी ट्रांसप्लांट किया गया। 

जानकारी के अनुसार 14 वर्षीय एक ब्रेन डेड बच्ची तीन लोगों को जिंदगी दे गई। बीती रात इस बच्ची की किडनी एसएमएस अस्पताल के गुर्दा रोग विभाग में भर्ती मरीज को ट्रांसप्लांट की गई।

बच्ची का लिवर बुधवार सुबह दिल्ली के आरआर अस्पताल को हवाई जहाज से भेजा गया है। तड़के 3.30 बजे किडनी दाता बच्ची को चिकित्सकों ने आपातकालीन इकाई से अंतिम विदाई दी। 

देर रात आई रिपोर्ट
एसएमएस अस्पताल प्रशासन के सूत्रों का कहना है कि बच्ची को मंगलवार को अस्पताल की कमेटी ने बे्रन डेड घोषित किया था। उसकी किडनी मैच के लिए एचएलए जांच के लिए नमूने दिल्ली भेजे गए थे। देर रात रिपोर्ट आने के बाद एक मरीज का कैडेबर किडनी ट्रांसप्लांट किया गया। 

तड़के तक चला ऑपरेशन
ट्रांसप्लांट रात 1.30 बजे शुरू होकर तड़के 4 बजे तक चला। मरीज को प्रत्यारोपण के बाद ट्रांसप्लांट वार्ड में शिफ्ट किया गया। अभी तक उसे पूरी तरह स्वस्थ्य बताया जा रहा है। यूरिन आउटपुट भी तय मानकों के अनुरूप आया है।
